As an auditor of medical records, I have come to judge the physician based on the record, and the record based on the minimum standards set forth in the Documentation Guidelines. Some physicians and non-physician practitioners (NPPs) believe that these guidelines were imposed upon them arbitrarily to make their lives difficult and to confuse them about the billing rules.
